Empowering Compassionate Care
Hospice of the Western Reserve is dedicated to providing exceptional patient care services, and we are seeking a skilled professional to lead our Hospice Home Care team.
Key Responsibilities:
- Coordinate clinical staff to deliver comprehensive, coordinated quality clinical care and service in homes.
- Evaluate facilitators of Care Conferences for Hospice Care Teams and provide guidance to Team Leaders.
- Supervise and develop Team Leaders to ensure effective patient care.
- Oversee compliance with Medicare and other licensure regulations.
- Maintain open communication and networking with all hospice community offices and coordinators.
- Participate in quality improvement and utilization management processes.
- Participate in on-call services as administrator on call.
- Fiscally responsible for managing the allocation of resources.
- Develop plans to operationalize programs and projects for the Home Care Programs.
- Conduct presentations and in-services in the community and Nursing Facilities to enhance education of hospice services.
Requirements:
- Five (5) year's experience in hospice home care management.
- Assertive and diplomatic in relating to people.
- Systematic and organized.
- Knowledgeable in hospice home care and community relations.
- Database literate.
- Comfortable and effective in relating to physicians and other health care professionals.
- Ability to successfully implement, monitor and evaluate programs.
- Ability to manage staff at multiple locations.
- Ability to manage conflict for positive results.
- Expertise in Medicare, Medicaid and JCAHO standards and regulations.

Physical Demands:
This is medium to heavy work requiring the exertion of up to 100 pounds of force occasionally and up to 20 pounds of force frequently, and the use of appropriate safety transfer techniques and body mechanics when moving, transporting, etc. of patients, sometimes in excess of 150 pounds.
