About the RoleWe're seeking a skilled Physical Therapist to join our team. As a Physical Therapist, you'll work closely with our interdisciplinary team to provide high-quality care to our patients. Your primary responsibility will be to provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
Key Responsibilities- Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
- Observe, record, and report patient reactions, changes, and progress to the attending physician and any appropriate agency staff.
- Counsel patients and their families on disease/injury management and proper use of equipment.
- Supervise therapy services provided by PTAs and nurse assistants, as needed.
- Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.

To be considered for this role, you'll need to have:
- A degree in Physical Therapy from an accredited program
- A valid state license to practice as a Physical Therapist
- Experience working in home care or a similar setting
