About the RoleWe're seeking a skilled and compassionate Physical Therapist to join our team. As a Physical Therapist, you'll work closely with patients to develop personalized treatment plans that help them achieve their goals and improve their overall health and well-being.
Key Responsibilities- Provide skilled physical therapy services to patients in their homes
- Develop and implement individualized treatment plans to meet patients' unique needs and goals
- Conduct thorough assessments and evaluations to identify patients' strengths and areas for improvement
- Collaborate with other healthcare professionals to ensure comprehensive care and support
- Document patient progress and outcomes in accordance with agency protocols and regulations
