About the RoleWe're seeking a skilled and compassionate Physical Therapist to join our team. As a Physical Therapist, you'll work closely with patients to develop and implement personalized treatment plans that help them achieve their goals and improve their overall health and well-being.
Key Responsibilities- Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician
- Observe, record, and report patient reactions, changes, and progress to the attending physician and other relevant staff members
- Develop and implement treatment plans that are tailored to each patient's unique needs and goals
- Collaborate with other healthcare professionals to ensure comprehensive and coordinated care
- Provide education and support to patients and their families to help them manage their conditions and achieve optimal health outcomes
