About the RoleWe're seeking a skilled and compassionate Physical Therapist to join our team. As a Physical Therapist, you'll work closely with patients to develop and implement personalized treatment plans that help them achieve their goals and improve their overall health and well-being.
Key Responsibilities- Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician
- Conduct thorough assessments and develop treatment plans that address patients' unique needs and goals
- Collaborate with other healthcare professionals to ensure seamless care and coordination
- Document patient progress and outcomes in accordance with agency protocol and Medicare/Federal guidelines
- Communicate effectively with patients, families, and healthcare teams to ensure optimal care and support
