About the RoleWe're seeking a skilled and compassionate Physical Therapist to join our team. As a Physical Therapist, you'll work closely with our interdisciplinary team to provide personalized care and support to patients in their homes. Your expertise and experience will be invaluable in helping patients achieve their goals and regain their independence.
Key Responsibilities- Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
- Observe, record, and report patient reactions, changes, and progress to the attending physician and other relevant staff members.
- Develop and implement individualized plans of care to meet the unique needs and goals of each patient.
- Collaborate with other healthcare professionals to ensure seamless care and support for patients and their families.
- Stay up-to-date with the latest clinical guidelines, best practices, and research in physical therapy to provide evidence-based care.

To be considered for this role, you'll need:
- A degree in Physical Therapy from an accredited program.
