About the RoleWe're seeking a skilled Physical Therapist to join our team. As a Physical Therapist, you'll work closely with our interdisciplinary team to provide comprehensive care to patients in their homes. Your primary responsibility will be to assess, plan, and implement individualized therapy programs to help patients achieve their goals and improve their overall health and well-being.
Key Responsibilities- Conduct thorough assessments of patients' physical abilities and develop personalized therapy plans to address their needs and goals.
- Provide skilled physical therapy services to patients in their homes, including exercises, stretches, and other interventions to promote mobility, strength, and flexibility.
- Collaborate with other healthcare professionals, including physicians, nurses, and therapists, to ensure comprehensive care and coordination of services.
- Document patient progress and outcomes, and communicate effectively with patients, families, and healthcare teams to ensure seamless care.
- Stay up-to-date with the latest evidence-based practices and guidelines in physical therapy, and participate in ongoing education and training to enhance your skills and knowledge.

To be considered for this role, you'll need:
- A Doctor of Physical Therapy (DPT) degree from an accredited program.
- A valid state license to practice as a Physical Therapist.
- Experience working with patients in a home care or rehabilitation setting.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
