Job Opportunity

We are currently seeking a skilled Community Disability Support Worker to join our team in Manchester, NH. This full-time role offers a unique opportunity to make a lasting impact on the lives of individuals with developmental disabilities.

In this position, you will provide direct support to individuals in their daily routines, including personal care, community engagement, and skill acquisition. You will also contribute to developing and implementing individualized support plans, ensuring seamless care coordination, and maintaining accurate documentation.

Key Responsibilities
  • Provide individualized support to individuals with developmental disabilities, focusing on building relationships, promoting social inclusion, and enhancing their quality of life.
  • Develop and implement individualized support plans, incorporating person-centered principles and behavioral strategies.
  • Maintain accurate and timely documentation, including progress notes, service plans, and data collection.
  • Collaborate with interdisciplinary teams to ensure comprehensive care coordination and communication.
  • Support individuals in accessing community resources, activities, and social opportunities.
Requirements
  • High School Diploma or equivalent required; experience in human services or related field desirable.
  • Demonstrate excellent communication, interpersonal, and problem-solving skills.
  • Able to work independently and as part of a collaborative team, adapting to changing circumstances.
  • Valid driver's license and reliable transportation essential; mileage reimbursement provided.
Salary and Benefits

This role comes with a competitive salary and benefits package, including:

  • $48,000 - $58,000 annual salary (commensurate with experience)
  • $1,000 sign-on bonus (split into two payments: $500 at hire and $500 after six months)
