Job Summary

The Mental Health Coordinator is a vital member of our team, responsible for assisting providers in an outpatient mental health setting. This role involves triaging and managing incoming referrals, providing case management services, and having some clinical responsibilities.

Key Responsibilities
  • Triage and manage referrals for the Mental Health Service Line
  • Input referrals into our medical record and tracking system
  • Provide non-contact clinical mental health interventions under direct supervision
  • Facilitate comprehensive care management related to bio-psychosocial components of illness, trauma, and disability
  • Assist providers with case management needs as requested
  • Support crisis intervention response
  • Complete screening tools as directed for front-end assessments
  • Maintain required records, statistics, and reports
  • Participate in weekly didactic, staff, and supervision meetings
Hospital Support
  • Facilitate bed searches and support to the mental health assessment team
  • Communicate with staff, patients, and families regarding progress on status of placement, discharge, etc.
  • Facilitate court hearings for patients who are being held under a TDO
  • Work within various systems to enter bed search updates, communication notes, and faxing referrals to outside hospitals, agencies
  • Provide expertise and engagement with families for mental health referrals and resources
  • Work in our scheduling system to get families scheduled with alternate levels of care
  • Participate in bi-monthly meetings with our clinical team
Requirements
  • Bachelor's degree from an accredited university is required
  • Qualified Mental Health Professional or at least one year of work experience in a mental health setting
  • Enrollment in a Master's Level Program from an accredited university with a degree in counseling, social, or psychology is preferred
  • Exceptional interpersonal, oral, and written communication skills necessary
  • Ability to work as a multidisciplinary team member
