Work Schedule: Monday - Friday, 8:00 AM - 4:30 PM


As an integral member of the Interdisciplinary Team (IDT), our Spiritual Care Providers are tasked with offering spiritual guidance and emotional assistance to both active and bereaved patients and their families. Our team is committed to the principles, objectives, and values inherent in hospice care.

Spiritual Care Providers are responsible for coordinating and executing all activities related to the spiritual dimension of the hospice program, aligning with organizational policies and procedures. Key Responsibilities:
- Evaluate the spiritual needs of patients and their families/caregivers in relation to the patient's terminal condition and surroundings. Share these insights with the RN Case Manager and interdisciplinary team.

- Provide holistic spiritual care to patients and families/caregivers of all faiths or none, as requested.

- Maintain ongoing communication with the Program Director to discuss clients and assist in addressing spiritual care requirements, including any unique or challenging client situations.

- Initiate contact with hospice staff and other facilities as necessary to ensure optimal care and resource utilization for patients and their families.


- Oversee and document clinical records to ensure compliance with regulatory standards regarding timeliness, accuracy, and completeness through electronic records, ensuring that issues, actions, and objectives are clearly articulated and updated as needed.


Qualifications:
- Graduate of an accredited seminary or theological institution, or possess relevant certification in pastoral ministry.

- A minimum of two units of Clinical Pastoral Education (four units preferred).

- At least two (2) years of experience as a healthcare pastor or chaplain.

- One (1) year of experience in hospice care is preferred but not mandatory.


- Must possess the ability to manage the emotional demands associated with providing care to patients and families facing pain, suffering, death, and grief.

Must be proficient in computer skills, including typing and familiarity with various software programs, particularly in electronic medical records.
