Position Overview

Salary Range: $23 - $28 per hour

Role Summary: The Senior Care Coordinator (SCC) plays a pivotal role in managing the care of clients and their families, ensuring they access and comprehend the various services and programs offered through the Enhanced Care Management (ECM) initiative. The SCC is dedicated to enhancing the health and overall quality of life of clients through our comprehensive services. This position requires a proactive individual who thrives in both collaborative and independent settings and fosters positive relationships with colleagues and external partners.


Key Responsibilities:

  • Case Management: Oversee a designated caseload of adult and pediatric clients, conducting thorough evaluations to identify their physical, emotional, and social requirements.
  • Care Strategy Development: Formulate personalized care strategies based on assessment results, taking into account medical history, preferences, and specific needs. Care strategies should be customized to align with individual goals.
  • Collaboration: Facilitate effective communication among healthcare professionals, social workers, therapists, and other members of the care team to ensure a holistic and integrated approach to client care. Work closely with Medical Doctors, Clinical Consultants, Housing Navigators, and Leadership to provide tailored recommendations.
  • Progress Monitoring: Regularly assess the advancement of clients and adjust care strategies as necessary, adhering to policy and compliance standards. Ensure that prescribed treatments and interventions are being followed, and communicate any significant changes in client status to primary care and specialty providers.
  • Client Engagement: Deliver exceptional client service through various support channels, including phone and in-person interactions. The Enhanced Care Management program emphasizes a high-touch, person-centered approach.
  • Record Keeping: Maintain precise and current documentation of assessments, care strategies, and client interactions. Ensure compliance with relevant regulations and standards, completing all required documentation accurately and promptly.
  • Advocacy: Act as a champion for clients, assisting them in navigating the healthcare landscape, understanding their treatment options, and accessing necessary services.
  • Educational Support: Provide informative resources to clients and their families regarding health-related topics, treatment alternatives, and self-care techniques.
  • Resource Connection: Identify and link clients with suitable community resources, support services, and programs to meet their needs, such as housing assistance, financial support, or counseling services.
  • Discharge Coordination: Assist in planning and coordinating the discharge process for clients transitioning from hospitals or long-term care facilities, ensuring a seamless move to home or another care environment.
  • Training Opportunities: Participate in the onboarding and training of new team members.
  • Additional Duties: Perform other responsibilities as assigned or required by departmental policies.

Qualifications:

  • Education & Experience: Associate Degree with a minimum of 2 years in healthcare or care coordination.
  • Language Proficiency: Fluent in English, both written and verbal.
  • Licenses/Certifications:
    • Valid Driver's License and proof of auto insurance.
    • Current BLS certification from the American Heart Association upon hire.
  • Required Skills:
    • Proficient in computer usage, email, virtual platforms, Excel, and other Microsoft Office applications.
    • Experience with Electronic Medical Records.
    • Strong verbal and written communication skills, with the ability to convey information clearly and effectively.
    • Problem-solving abilities, utilizing logic and relevant information to develop and implement solutions.
    • Capacity to work independently and complete assignments within established policies and procedures.

Work Environment:

  • This role offers a hybrid work schedule, with 30% of responsibilities performed remotely and 70% involving travel for in-person client visits. You will have flexibility in scheduling meetings with clients.
  • Eligible for mileage reimbursement for business-related travel.

Physical Demands: The position requires the ability to perform office tasks and drive, including but not limited to:

  • Consistently using a computer and other office equipment such as telephones, calculators, copiers, and printers.
  • Occasionally moving or carrying office equipment weighing up to 15 pounds.
  • Ability to operate a vehicle and travel to meet with assigned clients in the community, attend meetings, and visit various office locations as needed.
