Job Overview

POSITION SUMMARY

The Director of Integrated Health Services brings a robust background in operational management within healthcare settings, particularly in population health strategies. This role is pivotal in overseeing the daily functions of various Care Coordination teams and facilitating the rollout of innovative programs as determined by the Cypress Healthcare Partners Leadership Team. The Director will lead the development, implementation, and enhancement of population health services by managing a diverse team, including Registered Nurses, Licensed Vocational Nurses, Community Health Workers, and various coordinators focused on patient access and referrals.

The individual in this role is responsible for ensuring high-quality care for targeted patient groups as identified through strategic planning and defined processes. This position contributes significantly to the mission and strategic objectives of Cypress Healthcare Partners by spearheading initiatives aimed at refining and enhancing integrated services while ensuring compliance with relevant federal and state regulations, as well as local agency requirements. The Director will also be accountable for meeting programmatic standards set by various entities and payers.

KEY RESPONSIBILITIES

  • Oversees personnel management within the Care Coordination department, including recruitment, performance evaluations, and staff development.
  • Directs daily operations of the Care Coordination department and supports the introduction of new initiatives.
  • Demonstrates expertise in implementing best practices, achieving system objectives, and adhering to regulatory standards.
  • Collaborates with key stakeholders to drive improvements and implement necessary changes.
  • Maintains open communication with team members, continuously assessing plans to align with departmental goals and programmatic metrics.
  • Engages with multidisciplinary healthcare teams to ensure seamless care delivery, minimizing fragmentation and enhancing treatment plans.
  • Provides strategic and operational guidance to maximize the value of services delivered to patients and healthcare providers.
  • Leads efforts to attain recognition as Patient-Centered Medical Homes (PCMH) in accordance with NCQA standards.
  • Participates in initiatives related to value-based care and reimbursement models to understand programmatic goals and metrics.
  • Establishes and maintains best practice standards for population health workflows in a collaborative environment.
  • Develops and manages care management registries to address identified population health needs.
  • Monitors performance indicators and targets to enhance healthcare outcomes.
  • Evaluates metrics to promote individual and collective improvements in care across the organization.
  • Identifies workflow gaps and recommends enhancements based on comprehensive system knowledge.
  • Facilitates regular meetings with stakeholders and the Leadership Team.
  • Collaborates with analytics teams to provide insights that enable better patient care and cost management.
  • Participates in ongoing training and meetings to stay informed on population health and care coordination trends.
  • Ensures confidentiality in all aspects of patient and employee relations.
  • Performs additional duties as assigned.

EDUCATION AND EXPERIENCE

  • 5-7 years of experience in healthcare management, particularly in quality or population health management within a sizable medical practice.
  • Bachelor's degree required; advanced degrees in healthcare or business administration preferred.

REQUIREMENTS

  • Valid driver's license and reliable transportation.
  • Successful completion of a background check is required.

QUALIFICATIONS

  • Strong communication, interpersonal, and organizational skills.
  • Extensive experience with healthcare management systems.
  • Ability to adapt to changing priorities and work effectively in a team or independently.
  • Demonstrated initiative and a results-oriented approach.
  • Knowledge of clinical program development, project management, and the ability to influence stakeholders positively.
  • Familiarity with value-based payment systems.

PREFERRED SKILLS

Bilingual capabilities may be advantageous in certain roles.

SUPERVISORY RESPONSIBILITIES

This position supervises Practice Managers within Care Coordination and Population Health, overseeing a team of clinical and non-clinical staff.
