Position Overview

SUMMARY

The Head of Care Coordination at Cypress Healthcare Partners is responsible for overseeing operational functions within the organization, particularly in relation to population health management. This role entails directing and supervising the daily activities of various Care Coordination teams while facilitating the rollout of innovative programs as identified by the leadership team. The individual will provide strategic leadership in the development, implementation, and enhancement of healthcare services aimed at improving patient outcomes.

KEY RESPONSIBILITIES

  • Oversee personnel management within the Care Coordination department, including recruitment, performance evaluation, staff training, and disciplinary actions.
  • Direct daily operations of the Care Coordination department, ensuring the effective implementation of new initiatives.
  • Exhibit leadership capabilities by applying best practices, achieving system objectives, and adhering to regulatory standards.
  • Coordinate efforts with key stakeholders to implement changes that drive improvement in care delivery.
  • Maintain open communication with team members, continuously assessing departmental priorities and progress towards established goals.
  • Collaborate with multidisciplinary healthcare professionals to ensure seamless care transitions and minimize treatment plan discrepancies.
  • Provide clear and strategic guidance to enhance service delivery for patients, healthcare facilities, and providers.
  • Lead initiatives aimed at achieving recognition as a Patient-Centered Medical Home (PCMH) in accordance with established standards.
  • Engage in value-based contracting initiatives to understand programmatic goals and enhance patient care.
  • Develop and maintain best practice standards for population health workflows in alignment with an integrated team approach.
  • Optimize care management registries to meet the population health needs identified by the organization.
  • Establish and monitor performance metrics to improve healthcare outcomes.
  • Analyze metrics and benchmarks to foster improvement and promote collective growth within the Care Coordination department.
  • Identify workflow gaps and recommend enhancements based on comprehensive system knowledge.
  • Facilitate regular meetings with stakeholders and leadership to discuss progress and initiatives.
  • Work closely with the analytics team to generate reports that assist providers in addressing their patient populations effectively.
  • Participate in training and meetings to stay informed about developments in population health and care coordination.
  • Ensure compliance with regulations and maintain confidentiality in all patient and employee interactions.
  • Perform additional duties as assigned.

EDUCATION AND EXPERIENCE

  • 5-7 years of experience in a managerial role within a medical practice or in quality/population health management.
  • A Bachelor’s degree is required; a Master’s degree in healthcare or business administration is preferred.

QUALIFICATIONS

  • Exceptional communication, interpersonal, and organizational skills.
  • Proficient in using Epic software.
  • Ability to adapt to changing priorities with creativity and flexibility.
  • Strong initiative and capability to work both independently and collaboratively.
  • Demonstrated work ethic with a focus on innovation and results.
  • Understanding of clinical program development, analytical problem-solving, and project management.
  • Familiarity with value-based payment systems.

PREFERRED SKILLS

Bilingual capabilities may be required for certain roles within the organization.

SUPERVISORY ROLE

This position supervises Practice Managers within Care Coordination and Population Health, overseeing a team of clinical and non-clinical staff.
