Job Summary

We are seeking a skilled LPN to join our team as an After Hours LPN. As a key member of our hospice care team, you will provide high-quality patient care and support to patients and families in need.

Responsibilities
  • Conduct thorough nursing assessments to identify patient needs and develop effective care plans.
  • Provide compassionate and skilled care to patients in their homes, including medication management, wound care, and emotional support.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Document patient progress and care plans accurately and timely.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Diploma from a Board-approved nursing program.
  • Current, active, and unrestricted compact license as a Licensed Practical Nurse (LPN).
  • Minimum 1-year related experience in hospice care or a related field.
  • Computer proficiency and ability to navigate electronic medical records.
