Covenant Health is seeking a skilled LPN Home Health Nurse to join our team. As a LPN Home Health Nurse, you will provide high-quality nursing care to patients in their homes, working collaboratively with our interdisciplinary team to achieve positive patient outcomes.

Responsibilities:
  • Perform and record cardiovascular assessments, pulmonary assessments, neurological assessments, GI assessments, skin assessments, intake and output assessments, and pain assessments.
  • Administer medications per HomeCare policy, clearly documenting administration and checking for allergies and MD orders.
  • Insert nasogastric/gastrostomy tubes, replace gastrostomy tubes, and provide teaching/training for patient/caregiver on care of feeding tube.
  • Perform intravenous procedures, including inserting peripheral IV lines, calculating and maintaining correct rate of IV infusion, labeling, and changing tubing.
  • Provide colostomy care, tracheostomy care, and suctioning.
  • Work collaboratively with members of the team to achieve positive patient care outcomes.
  • Perform basic infection control techniques, including proper hand-washing, glove usage, and cleaning equipment.
Requirements:
  • Tennessee LPN license or Licensed Vocational Nurse.
  • One (1) year of professional experience in hospital or prior home health experience.
  • Successful completion of LPN advanced skills course.
  • CPR certification.
  • Valid Tennessee driver's license and state mandated minimum insurance coverage.
