As a Home Health LPN, you will play a vital role in delivering high-quality patient care in the comfort of their own homes. Our team is dedicated to providing personalized, comprehensive care that addresses the physical, emotional, and social needs of our patients.

  • Provide professional nursing services under the direction of a RN, ensuring compliance with state regulations, healthcare standards, and CenterWell Home Health policies.
  • Develop and implement individualized care plans that promote patient independence, rehabilitation, and overall well-being.
  • Collaborate with our interdisciplinary team to ensure seamless care coordination and effective communication with patients, families, and healthcare providers.
  • Utilize your clinical expertise to assess patient needs, identify areas for improvement, and implement evidence-based interventions.
  • Document patient progress, outcomes, and any changes to care plans in a timely and accurate manner.
  • Participate in ongoing education and training to stay current with best practices, industry developments, and regulatory requirements.

Requirements and Qualifications

  • Graduate of an accredited Licensed Practical Nursing program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid driver's license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
