Registered Nurse Care Coordinator

We are looking for a dedicated Registered Nurse Care Coordinator to join Patient's Choice Home Health Care, a branch of Nomi Health. In this role, you will report directly to the Director of Nursing.

Your primary responsibility will be to oversee the entire patient care process, which includes managing and supervising home care staff.

You will cultivate and sustain positive relationships with patients and referral sources while addressing any inquiries or concerns they may have.

Our services encompass skilled nursing, physical and occupational therapy, personal care, and various home health solutions throughout the region, always prioritizing quality care and respect for our patients.

Key Responsibilities:

  • Direct daily patient care assignments and collaborate effectively with field staff.
  • Ensure comprehensive patient needs are met and assist with community transfers and referrals to other providers, following up to confirm completion.
  • Maintain accurate documentation of all activities in client files, including ordering durable medical equipment and supplies as necessary.
  • Implement enhanced work methods and procedures to improve service delivery.
  • Ensure patient admissions, recertifications, and discharges comply with established policies.
  • Foster and maintain strong working relationships with current and potential referral sources.
  • Conduct case conferences to discuss patient progress, needs, and any relevant concerns regarding care.
  • Promote and monitor community and patient perceptions of the agency as a provider of high-quality services.
  • Ensure supervisory visits are conducted in line with guidelines.
  • Assume responsibility for agency operations in the absence of the Administrator and Director of Nursing, if requested.
  • Uphold confidentiality for both patients and the agency.
  • Assist with additional duties as needed.
Qualifications:

  • High school diploma or equivalent.
  • Valid Registered Nurse (RN) license.
  • Minimum of 1 year of experience in a healthcare environment, preferably in home health.
  • Prior experience in case management or a related field is preferred.
  • Familiarity with medical terminology, social service systems, and healthcare resources.
  • Exceptional interpersonal and communication skills to establish rapport with clients and collaborate with interdisciplinary teams.
  • Ability to empathize with clients' challenges and provide compassionate support.
  • Strong problem-solving abilities to assess client needs and create effective care plans.
  • Proficient in managing multiple cases, deadlines, and documentation efficiently.
