Key Responsibilities:
- Employs the nursing process to assess, plan, implement, and evaluate care for patients receiving contracted services.
- Initiates and adjusts care plans as directed, collaborating with prescribers to adapt to changing patient needs.
- Integrates therapeutic, preventive, and rehabilitative nursing interventions based on patient assessments and goals.
- Responds to changes in patient conditions and communicates findings to the appropriate healthcare provider.
- Administers prescribed medications and treatments, monitoring their effectiveness and documenting outcomes.
- Evaluates care plans and identifies educational needs for patients and families, focusing on care coordination and discharge planning.
Qualifications:
- Active Registered Nurse (RN) License in the relevant state.
- Valid Health Certificate as per facility requirements.
- Current PPD or Chest X-Ray documentation.
- Up-to-date BLS certification.
- Preferred: Minimum of one year of prior RN experience.
