Job Description:

We are seeking a skilled Nurse Care Coordinator to join our team at Community Options. As a key member of our healthcare team, you will play a vital role in delivering high-quality care to individuals with disabilities.

Responsibilities:
  • Develop and implement individualized care plans in collaboration with healthcare providers and support staff.
  • Conduct regular assessments and evaluations to ensure optimal health outcomes.
  • Collaborate with community resources and service providers to ensure seamless care delivery.
  • Provide education and training to support staff on medical procedures and best practices.
  • Maintain accurate and timely records of patient care and progress.
Requirements:
  • Bachelor's Degree in Nursing or related field.
  • Minimum one year of experience in nursing, preferably in a community-based setting.
  • Valid RN license and current CPR certification.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
Salary and Benefits:
