Job Summary

We are seeking a skilled Case Manager to join our team at Delta-T Group Inc. as a Behavioral Health Specialist. This is a temporary to permanent role that requires a strong background in human services and experience in case management.

Key Responsibilities
  • Provide case management and assessment services to consumers in an on-site setting.
  • Coordinate service delivery and ensure successful client linkage with other programs and facilities.
  • Develop and maintain a caseload of case management clients.
  • Document case management progress notes in a timely fashion.
  • Complete initial applications of referrals on a scheduled basis.
  • Plan, structure, and facilitate groups to support skill acquisition.
  • Provide outreach for consumers who have missed their appointments.
  • Attend multi-disciplinary team meetings to provide input and obtain feedback about consumer cases.
  • Develop community contacts to secure community referrals and ensure consumers' needs are met.
  • Utilize a person-centered approach to aid consumers in meeting their prioritized needs.
  • Attend client trainings as appropriate to the position or for requirements for the agency.
Requirements
  • Bachelor's Degree in Human Services or a related field.
  • 2 years of work experience in case management or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Flexibility to accommodate a variety of schedules and client needs.
