Position Overview

We are offering a competitive sign-on bonus of $7,500 for this full-time day position. Compensation

  • Annual salary range: $59,280 to $71,760

Responsibilities
  • Conduct patient assessments and collaborate with the care team to formulate and execute individualized care plans.
  • Refer patients to other healthcare professionals as necessary based on identified needs or document reasons for not doing so.
  • Encourage patient health and independence through education and appropriate rehabilitative strategies, aiding patients in mastering self-care techniques.
  • Supervise LPNs and HHAs.
  • Ensure timely and accurate documentation of patient information.
  • Regularly update the clinical manager and care team on patient progress.
  • Plan and deliver staff training and education.
  • Fulfill on-call responsibilities and provide on-call services to patients and families as required.
  • Engage in clinical development and continuing education initiatives.
  • Perform other related duties as assigned.

Qualifications
  • Minimum of one (1) year of clinical experience as a Registered Nurse (RN). Candidates with less than one year of experience must receive approval from the VP Clinical.
  • Current RN license specific to the state(s) of employment.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
