Position Overview
We are offering a competitive sign-on bonus of $7,500 for this full-time daytime role. Compensation and Benefits
Our compensation package is attractive and includes:
- Annual salary ranging from $59,280 to $71,760.

Key Responsibilities
- Conduct patient assessments and collaborate with the care team to create and implement personalized care plans.
- Make referrals to other healthcare professionals as necessary based on patient needs.
- Promote patient health and independence through education and appropriate rehabilitative measures.
- Supervise LPNs and HHAs to ensure quality care delivery.
- Maintain accurate and timely documentation of patient care.
- Regularly communicate patient progress to the clinical manager and care team.
- Plan and provide educational opportunities for staff.
- Participate in on-call responsibilities as assigned.
- Engage in clinical development and continuing education initiatives.
- Perform additional duties as required.
- Minimum of one year of clinical experience as a Registered Nurse (RN). Candidates with less than one year of experience must receive approval from the VP Clinical.
- Current RN license specific to the state of employment.
- Current CPR certification.
- Valid driver's license, reliable transportation, and liability insurance.
