Position Overview

We are offering a competitive sign-on bonus of $7,500 for this full-time daytime role. Key Responsibilities
  • Conduct patient assessments and collaborate with the care team to create and implement personalized care plans.
  • Make referrals to other healthcare professionals as necessary based on patient needs.
  • Promote patient health and independence through education and appropriate rehabilitative measures.
  • Supervise LPNs and HHAs to ensure quality care delivery.
  • Maintain accurate and timely documentation of patient care.
  • Regularly communicate patient progress to the clinical manager and care team.
  • Plan and provide educational opportunities for staff.
  • Participate in on-call responsibilities as assigned.
  • Engage in clinical development and continuing education initiatives.
  • Perform additional duties as required.
Qualifications
  • Minimum of one year of clinical experience as a Registered Nurse (RN). Candidates with less than one year of experience must receive approval from the VP Clinical.
  • Current RN license specific to the state of employment.
  • Current CPR certification.
  • Valid driver's license, reliable transportation, and liability insurance.
