Ambulatory Clinical Pharmacist

Ochsner Health System is seeking a highly skilled Ambulatory Clinical Pharmacist to join our team. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and contributing to the success of our organization.
Key Responsibilities- Provide pharmacokinetic consultation and use laboratory data to evaluate drug selection and dosing
- Evaluate patient response to drug therapy and document interventions
- Develop and implement pathways, best practices, and guidelines for assigned populations
- Educate staff, students, patients, and healthcare providers on medication management and clinical activities
- Mentor residents, students, and new staff to meet departmental goals
- Current Pharmacy license in the state of practice
- Completion of a PGY1 Pharmacy Practice Residency (3 years related specialty experience accepted in lieu of PGY1 Residency)
- Preferred: Doctor of Pharmacy (PharmD) and Completion of a PGY2 Pharmacy Residency
