Ambulatory Clinical Pharmacist

We are seeking a highly skilled and dedicated Ambulatory Clinical Pharmacist to join our team at Ochsner Health System. As a key member of our healthcare team, you will play a vital role in providing exceptional patient care and contributing to the success of our organization.
Responsibilities- Provide pharmacokinetic consultation and use laboratory data to evaluate drug selection and dosing, communicating findings to healthcare practitioners.
- Evaluate patient response to drug therapy and document interventions, conducting outcome studies and evaluations to ensure good patient outcomes.
- Assist in the development of pathways, best practices, and guidelines for assigned patient populations.
- Educate staff, students, patients, healthcare providers, and other healthcare professionals, submitting publications as needed.
- Mentor residents, students, and new staff to meet departmental goals.
- Take responsibility for overall patient care beyond hospital visits, through all transitions of care.
- Accommodate special needs of the organization by volunteering to perform tasks during staffing shortages.
- Adapt behavior to specific patient populations, respecting privacy and adapting communication styles.
- Bachelor of Science in Pharmacy (BSPharm) or Doctor of Pharmacy (PharmD) degree.
- Completion of a PGY1 Pharmacy Practice Residency or 3 years of related specialty experience.
- Current Pharmacy license in the state of practice.
- Strong leadership and teaching skills, with effective verbal and written communication skills.
- Proficiency with Windows-style applications and keyboard.
The incumbent works with patients who have known or suspected communicable diseases and may enter isolation rooms. The incumbent has an occupational risk for exposure to all communicable diseases. There is potential to encounter hazardous medications and hazardous waste.
