We are seeking a highly motivated and creative Care Coordinator to join our Community Habilitation services in the Hudson Valley. As a key member of our team, you will play an integral role in supporting individuals with I/DD in achieving their goals and maximizing independence.

**Job Summary:**
We are looking for a skilled Care Coordinator to provide one-on-one supports to individuals with I/DD in community and in-home settings. Your primary responsibilities will include:

  • Providing personalized support and teaching life skills to individuals with I/DD
  • Transporting and escorting individuals on recreational activities, medical appointments, and home visits
  • Building positive relationships and maintaining a therapeutic environment
  • Completing documentation and ensuring compliance with regulatory requirements

**Required Skills and Qualifications:**
To be successful in this role, you will need:

  • A High School Diploma or GED
  • Work or life experience within the intellectual/developmental disabilities field (a plus)
  • Basic written and verbal communication skills
  • Basic computer knowledge
  • A valid Driver's License (may be required depending on the position)
