We are seeking a dedicated Nursing Care Professional to join our team at Northwell Health. This is an exciting opportunity for a Registered Nurse to provide exceptional patient care and work collaboratively with our healthcare team.
Job Summary:
The successful candidate will be responsible for delivering high-quality nursing care to designated patients, utilizing the nursing process of assessment, diagnosis, outcome identification, planning, implementation, and evaluation of patient care. Collaborating with physicians and other healthcare team members in coordinating and implementing procedures and treatments will be essential to this role.
Key Responsibilities:
- Conduct thorough patient interviews, explain policies and procedures, review patient charts, and answer questions correctly and courteously.
- Assess gastrointestinal, cardiovascular, respiratory, renal, and neurological health status.
- Determine mobility, sensory deficits, prostheses use, and skin condition and adjust the plan of care as necessary.
- Communicate and document patient physiological health status and care plans.
- Assess patient psychosocial health status.
- Elicit perceptions of medical/nursing care and expectations of care.
- Determine coping mechanisms, knowledge levels, and ability to comprehend.
- Communicate and document psychological status and support groups, counseling services, and social services.
- Identify patient outcomes.
- Develop criteria for measuring patient outcomes.
- Identify actual/potential patient problems.
- Develop patient outcome statements and establish individualized patient goals.
- Identify care activities and establish priorities to achieve expected outcomes.
- C coordinated cost-effective use of supplies, equipment, and medication to achieve expected outcomes.
- Document the plan of care and collaborate with physicians and other healthcare team members.
- Implement the plan of care.
- Maintain constant vigilance over patient care to ensure safety precautions/needs are followed.
- Provide emotional support to patients and significant others.
- Reassess patients as needed and revise the plan of care accordingly.
- Administer prescribed treatments correctly.
- Use necessary equipment correctly for patient care.
- Provide discharge instructions to patients and significant others.
- Evaluate care provided for patient outcomes.
- Document patient treatment, activity, and condition.
- Use appropriate documentation methods according to departmental policy.
- Collaborate with other care team members in planning and carrying out treatment regimens.
- Direct other members of the care team.
- Perform Decubitus Care (I-IV).
- Administer medications correctly and safely.
- Explain patient conditions to them as needed.
- Communicate assessment data in an orderly fashion by recording, updating, and verbalizing pertinent information to care team members and appropriate agencies.
- Evaluate teaching effectiveness by feedback from patients/families and document same.
Requirements:
- Graduate from an accredited School of Nursing required.
- Bachelor's Degree in Nursing preferred.
- Must be enrolled in an accredited BSN program within two years and obtain a BSN Degree within five years of job entry date.
- Current License to practice as a Registered Professional Nurse in New York State required.
- Specialized certifications as needed.
- AHA BLS, ACLS, & PALS required.
Salary Estimate:
$110,000 - $140,000 per year based on experience and qualifications.
