Overview

We are seeking a highly skilled Registered Nurse Case Manager to join our team at Bridge Home Health and Hospice. As a RN Case Manager, you will play a critical role in providing high-quality patient care and coordinating services to meet the needs of individuals and families within their homes and communities.

Responsibilities
  • Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
  • Provide professional nursing care by utilizing the nursing process and adhering to the state Nurse Practice Act.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Supervise and delegate responsibilities to ancillary personnel as needed.
  • Participate in quality assessment and performance improvement activities to ensure high-quality patient care.
Requirements
  • A graduate of a National League of Nursing accredited school of nursing.
  • Current licensure in the State of California and current CPR certification.
  • A minimum of one year of experience as a professional nurse within the last three years or a baccalaureate degree in nursing from a program accredited by the National League for Nursing.
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
