Felton Institute is seeking a skilled Clinical Case Manager to join our team of dedicated professionals. As a Clinical Case Manager, you will play a critical role in providing recovery-oriented, culturally responsive, and trauma-informed specialty behavioral health services to clients 18 and older.

The ideal candidate will have a Master's Degree in Social Sciences, Psychology, Counseling Psychology, or a related field, and current California State Licensure or registration as an associate. Previous experience in similar settings, including mental health services, substance use disorder treatment, and homelessness, is highly desirable.

Key Responsibilities:

  • Provides trauma-informed, culturally responsive services, including mental health services, contingency management, motivational interviewing, and crisis intervention.
  • Elicits and maintains strong working relationships with clients and the care team, including other professionals, peers, medical providers, and client-identified support persons.
  • Facilitates and tracks linkages, care coordination, and case management among medical and community-based supports.
  • Outreaches to shelters, streets, hospitals, and community settings to meet clients where they are at and able to locate clients using electronic health record systems and planning with clients.
  • Ensures timely and accurate documentation of all services provided and maintains applicable chart requirements.
  • Provides inter-agency collaboration to facilitate problem-solving and clinical care.
  • On-call consult during working hours to triage clinical emergencies and coordination for coverage to the team.
  • Serves as a back-up for the team as needed.
  • Attends and participates in all assigned trainings for professional, program, and agency development.
