Job Summary

We are seeking a skilled Nurse Case Manager I to join our team at Partnership HealthPlan of California. This role is responsible for coordinating and managing the care of our members, ensuring they receive the best possible outcomes.

Key Responsibilities
  • Initiate and coordinate individualized care plans for assigned members, addressing both clinical and non-clinical components.
  • Collaborate with multidisciplinary teams to develop and implement care plans that meet the comprehensive medical, behavioral, and psychosocial needs of members.
  • Provide case management services independently for a caseload ranging in complexity from basic to complex.
  • Communicate effectively with members, providers, vendors, community partners, and PHC employees to ensure a smooth implementation and continuum of care.
  • Maintain accurate and timely documentation, records, and case files in the PHC Case Management System.
Requirements
  • Associate's degree in Nursing required; Bachelor's degree in Nursing (or higher) preferred.
  • 2 years of experience preferred, to include at least one (1) year of case management experience and one (1) year in an acute care setting.
  • Current and unrestricted California Registered Nurse License.
  • Valid California driver's license and proof of current automobile insurance compliant with PHC policy.
