Dietitian - Nutrition Specialist

New Horizon Foods is seeking a highly skilled and experienced Dietitian to join our team as a Senior Nutrition Care Coordinator. The successful candidate will be responsible for providing comprehensive nutrition care plans to our senior residents living in multiple locations.
Key Responsibilities- Develop and implement personalized nutrition care plans for senior residents, taking into account their unique needs and health requirements.
- Conduct thorough assessments of residents' nutritional needs and provide evidence-based recommendations to ensure optimal health outcomes.
- Collaborate closely with other healthcare professionals, including physicians, nurses, and therapists, to ensure that residents receive comprehensive and coordinated care.
- Educate residents and their families on healthy eating habits, nutrition, and wellness strategies to promote overall health and well-being.
- Monitor and evaluate the effectiveness of nutrition care plans, making adjustments as needed to ensure optimal outcomes.
- Maintain accurate and up-to-date records of resident nutrition assessments and care plans, ensuring compliance with regulatory requirements.
- Proven experience as a Dietitian, with a strong background in nutrition and healthcare.
- Experience working with seniors in a senior living environment, with a focus on providing high-quality care and services.
- Strong knowledge of nutrition, healthy eating habits, and wellness strategies, with the ability to communicate complex information to residents and their families.
- Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals.
- Ability to work independently and as part of a team, with a focus on providing exceptional customer service and support.
- Bachelor's degree in Nutrition or a related field, with a strong academic record and relevant coursework.
- Registered Dietitian (RD) certification, with a strong commitment to ongoing professional development and continuing education.
