The Home Care Coordinator is integral to this mission, responsible for performing in-home care evaluations and establishing the foundation for our home health team to support our participants effectively.
Reporting directly to the Home Health Manager, the Home Care Coordinator's primary focus is on organizing, evaluating, and supervising personal care services within the home environment.
Key Responsibilities:
- Manage and coordinate incoming inquiries from participants, healthcare providers, and agency services concerning physician directives, participant inquiries, and referrals.
- Engage with participants through phone communication, ensuring effective collaboration with nursing, therapy, aides, social services, and physicians regarding any changes in participant or staff schedules, test outcomes, etc.
- Collaborate with Home Care Services personnel to monitor and track home care schedules and hours.
- Work alongside the Marketing Team to facilitate the enrollment of potential participants into the program.
- Assist in staffing and scheduling tasks, gathering input from management.
- Contribute to end-of-life care coordination and support.
Qualifications:
- Healthcare/Medical Licensure or equivalent; with a minimum of three (3) years of relevant professional experience.
- Bachelor's Degree is preferred.
- At least three (3) years of experience in case management or nursing within a clinical or home care setting, particularly with frail or elderly populations.
- Preferred nursing knowledge and training to address the needs of frail elderly participants and manage complex clinical conditions.

Compensation for this role ranges from $67,829 to $81,395 annually, plus bonuses and equity. Actual compensation will be adjusted based on experience and qualifications.
