We are seeking a skilled Clinical Care Coordinator to join our team at Weill Cornell Medicine. As a key member of our healthcare team, you will be responsible for coordinating and organizing the clinical care of patients within our practice.

Responsibilities
  • Patient Coordination: Coordinate and organize the clinical care of patients within our practice, ensuring timely and effective delivery of services.
  • Clinical Support: Provide in-office patient care services, including obtaining patient histories, vital signs, venipuncture, specimen collection, administering vaccines/medications, and educating patients.
  • Inventory Management: Maintain inventory of supplies, equipment, and reagents, ordering as necessary to ensure smooth functioning of the practice.
  • Administrative Tasks: Schedules tests and procedures for patients, coordinates follow-up care, and records diagnosis, treatment plans, progress notes, and doctors' orders in patient charts.
Requirements

To be considered for this role, you must have a current NY State Registered Nurse License & Registration, BLS Certification (issued by the American Heart Association), and an Infection Control Certificate. A Bachelor's Degree in Nursing is preferred. Up to 2 years of post-degree course work and/or an internship and less than one year of previous experience may be considered.
