We are seeking a skilled Clinical Transplant Coordinator to join our team at the Mount Sinai Health System. As a key member of our transplant program, you will be responsible for coordinating clinical programs and services for designated patient populations.

Job Description:

Responsibilities:

  • Coordinates the multidisciplinary screening and evaluation of patients seen in the transplant practice at MSH and approved satellites.
  • Ensures all necessary lab tests are performed on potential transplant candidates and that results are updated as required.
  • Participates in making recommendations regarding placing patients on the transplant waiting list.
  • Communicates with all active transplant candidates regarding waiting list status on an ongoing basis.
  • Fulfills on-call responsibilities, including facilitating organ offers from UNOS and communicating with surgeons, fellows, and patients.
  • Follows patients during their hospitalization and coordinates consulting services as needed.
  • Educates patients and families regarding transplant medications, lifestyle changes, and outpatient follow-up protocols.
Qualifications:

Requirements:

  • NY RN with Baccalaureate Degree in Nursing is required.
  • Masters in Nursing and certification in specialty area are preferred.
  • At least 2 years acute care nursing, critical care nursing preferred.
Compensation:

The estimated annual salary for this position is $120,000 - $150,000, depending on experience and qualifications.
