Our sub-acute floor is a 59-bed Medicare unit that provides care to residents with multiple clinical issues, including CHF, dehydration, cellulites, diabetes, DVT, and stable cardiac-related issues. Our team works closely with healthcare professionals, including doctors, social workers, dietitians, PT/OT, hospice, and spiritual support, to provide comprehensive care.

Our Chronic Care FloorOur 60-bed Medicare/Rehabilitation and long-term care unit is a great place to learn basic med-surg, I.V. therapy, and wound treatments nursing skills. Our team works closely with healthcare professionals to provide and implement the best treatment plan for residents.
This fast-paced and busy unit provides a great atmosphere for PT/OT residents to be motivated and reach their goals.
Our Dementia Care FloorOur 60-bed Medicare/Medicaid/Dementia unit provides care to residents with a variety of diagnoses. We create a fullness of life through the joy of relationships, the art of caring, and the spirit of living.
This unit works closely with a multidisciplinary team to discuss and implement the resident's plan of care.
About the FacilityBeaumont Commons – Farmington Hills is a 179-bed facility with three floors and nine units. We provide care for long-term care residents and post-acute rehab patients.
Our first floor has 59 beds with a focus on sub-acute rehab, the second floor has 59 beds with a focus on long-term care and sub-acute rehab, and our third floor is a secured dementia unit.
Nurse-patient ratios vary depending on the shift and the population you will be serving. We care for dementia residents, long-term care, and sub-acute rehab residents.
We accept patients with specialties such as general rehab, neuro rehab, cardiac, respiratory, and cancer. We also have various programs for residents to participate in multiple activities.

Scope of WorkAs a Licensed Practical Nurse (LPN), you will contribute to the assessment, planning, implementation, and evaluation of patient care under the direction of the RN/Mid-level Provider/Physician and in collaboration with other members of the healthcare team.
You will inform patients and families about treatments and medications being received, equipment being utilized, and other related activities as appropriate, in coordination with the RN/Mid-level Provider/Physician.
You will document care provided via computer/written patient record in a manner consistent with guidelines.
