Certified Nursing Assistant Long Term Care Specialist

Our sub-acute floor is a 59-bed Medicare unit that caters to residents with complex clinical issues, including congestive heart failure, dehydration, cellulitis, diabetes, deep vein thrombosis, and stable cardiac conditions requiring monitoring.
Nurses collaborate closely with a multidisciplinary team, including doctors, social workers, dietitians, physical therapists, occupational therapists, hospice staff, and spiritual support professionals.
Our sub-acute unit offers a well-rounded opportunity for learning, professional growth, and personal satisfaction.
Our Chronic Care floor is a 60-bed Medicare/Rehabilitation and long-term care unit, providing a great platform to develop basic med-surg, IV therapy, and wound treatment nursing skills.
Nurses on this unit work alongside healthcare professionals to implement the best treatment plans for residents.
This fast-paced and dynamic unit offers a motivating environment for physical therapy and occupational therapy residents to achieve their goals.
About the FacilityBeaumont Commons – Farmington Hills is a 3-floor facility with 9 units, offering care for long-term care residents and post-acute rehab patients.
The first floor has 59 beds, focusing on sub-acute rehab, while the second floor has 59 beds, focusing on long-term care and sub-acute rehab.
The third floor is a secured dementia unit.
Nurse-patient ratios vary depending on the shift and population served.
We care for dementia residents, long-term care residents, and sub-acute rehab residents.
We accept patients with various specialties, including general rehab, neuro rehab, cardiac, respiratory, and cancer.
We offer various programs for residents to participate in multiple activities.

Scope of WorkActs as a team member in delivering quality, direct patient care.
Performs basic skills per department guidelines and communicates observations to the healthcare team.
Provides direct patient care under the direction of the Unit Manager or Licensed Nurse, consistent with policies and procedures.
Responsible for direct day-to-day care of assigned residents/patients, including dressing, bathing, transporting, and feeding.
Documents care provided as directed in a manner consistent with guidelines.

QualificationsRequired: High School Diploma or equivalent.
