LPN Flex Long Term Care Opportunity

Our sub-acute floor is a 59-bed Medicare unit that provides care to residents with multiple clinical issues, including CHF, dehydration, cellulites, diabetes, DVT, and stable cardiac-related issues. Nurses work closely with a variety of healthcare professionals, including doctors, social workers, dietitians, PT/OT, hospice, and spiritual support.
Our sub-acute unit offers a well-rounded opportunity for learning, professional growth, and personal satisfaction. Nurses on this unit work closely with a multidisciplinary team to provide and implement the best treatment plan for residents.
About the FacilityBeaumont Commons – Farmington Hills is a 179-bed facility that provides long-term care and post-acute rehab services. The facility has three floors, each with nine units, and offers a range of programs and activities for residents.
The first floor has 59 beds and focuses on sub-acute rehab, while the second floor has 59 beds and focuses on long-term care and sub-acute rehab. The third floor is a secured dementia unit.
Scope of WorkThe Licensed Practical Nurse (LPN) will work under the direction of the RN/Mid-level Provider/Physician and in collaboration with other members of the healthcare team to contribute to the assessment, planning, implementation, and evaluation of patient care.
The LPN will inform patients and families about treatments and medications being received, equipment being utilized, and other related activities as appropriate, in coordination with the RN/Mid-level Provider/Physician.
The LPN will document care provided via computer/written patient record in a manner consistent with guidelines.
