Nurses work closely with a variety of healthcare professionals, including doctors, social workers, dietitians, PT/OT, hospice, and spiritual support.

Our sub-acute unit offers a well-rounded opportunity for learning, professional growth, and personal satisfaction.

Our Chronic Care floor is a 60-bed Medicare/Rehabilitation and long-term care unit, providing a great place to learn basic med-surg, I.V. therapy, and wound treatments nursing skills.

Nurses on this unit, along with a host of healthcare professionals, work to provide and implement the best treatment plan for residents.

This fast-paced and busy unit provides a great atmosphere for PT/OT residents to be motivated and reach their goals.

About the Facility

Beaumont Commons Farmington Hills is a 3-floor facility with 9 units, providing care for long-term care residents and post-acute rehab patients.

The first floor has 59 beds with a focus on sub-acute rehab, the second floor has 59 beds focusing on long-term care and sub-acute rehab, and our third floor is a secured dementia unit.

Nurse-patient ratios vary depending on the shift and the population being served.

We care for dementia residents, long-term care, and sub-acute rehab residents.

With 179 beds, we accept patients with specialties such as general rehab, neuro rehab, cardiac, respiratory, and cancer.

We also have various programs for residents to participate in multiple activities.

Our goal is to create a Fullness of Life through the joy of relationships, the art of caring, and the spirit of living.

Scope of Work

Licensed Practical Nurse (LPN)

Under the direction of the RN/Mid-level Provider/Physician and in collaboration with other members of the healthcare team, contributes to the assessment, planning, implementation, and evaluation of patient care.

Informs patients and families about treatments and medications being received, equipment being utilized, and other related activities as appropriate, in coordination with the RN/Mid-level Provider/Physician.

Documents care provided via computer/written patient record in a manner consistent with guidelines.

Qualifications
  • Required: Graduate of an approved LPN program.
  • Experience in Skilled Nursing/Long Term Care preferred.
  • License: License Practical Nursing (LPN) - STATE_MI State of Michigan Upon Hire required.
  • Certification: Basic Life Support (BLS) - AHA American Heart Association Upon Hire required.
