About the Role

We are seeking a highly skilled Social Worker BSW to join our team at Corewell Health. As a Social Worker BSW, you will play a vital role in providing and coordinating medically related social services to attain and maintain the highest practicable physical, mental, and psychosocial well-being of each resident.

Key Responsibilities:

  • Provide and coordinate medically related social services to residents
  • Assess and develop resident care plans
  • Maintain contact with residents and their family members
  • Ensure all charted progress notes are informative and descriptive of the services provided

Qualifications

Required:

  • Associate's Degree in Social Work or a related field
  • Minimum of an associate's degree in social work in a human services field

Preferred:

  • Bachelor's Degree in Social Work
  • LIC-Bachelor Social Worker (BSW) - STATE_MI State of Michigan Upon Hire preferred
