Job Summary

We are seeking a highly skilled School Based Behavioral Health Liaison to join our team at Swope Health Services. This role will provide counseling and case management services to clients and their families, working collaboratively with assigned schools to deliver comprehensive care and treatment planning.

Key Responsibilities
  • Intake Assessments: Perform initial intake assessments, 6-month reviews, and annuals as needed to address specific needs.
  • Chart Documentation: Complete chart documentation reviews, including Psychosocial History, Treatment Planning, Chart Progress Notes, Quarterly Reviews, and Discharge Summaries within established guidelines and time frames.
  • Counseling Services: Provide individual, family, and group counseling using approved evidence-based treatment approaches sanctioned by the Swope Behavioral Health Department.
  • Treatment Team Meetings: Participate in treatment team meetings with school staff to establish treatment goals for a comprehensive treatment plan.
  • Case Management: Offer brief-targeted case management services to assist with coordinating student support and connection to community resources for holistic care.
  • Mental Health Crisis Situations: Be available to assist in mental health crisis situations as needed to ensure immediate care and support.
  • Billing Documentation: Accurately report billing documentation consistent with the requirements of the particular payer or contract.
  • Productivity Guidelines: Meet expected productivity guidelines and standards.
  • Continuous Performance Improvement: Participate in Continuous Performance Improvement activities.
  • Diagnostic Formulation: Possess and demonstrate expertise around diagnostic formulation as it relates to the most current DSM.
Requirements
  • Master's Degree: Master's degree in Social Work, Psychology, Counseling and Guidance, or a related field.
  • Preferred Licenses: Provisionally Licensed Master Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage and Family Therapist (LMFT).
  • Experience: 1-3 years of prior experience with children/youth with serious emotional disturbances or 3-5 years of prior related experience.
  • Knowledge: Working knowledge of the current DSM and ability to assess mental health disorders, knowledgeable regarding trauma-informed care, and knowledge of theory and practice of mental health assessment and treatment.
