Primary Care Physician Opportunity at Yale New Haven Health

We are seeking an experienced Primary Care Physician to join our team at Yale New Haven Health. As a Primary Care Physician, you will be responsible for providing high-quality, patient-centered care to our patients in Greenwich, CT.

About the Role

This is a full-time opportunity to work in a dynamic and collaborative environment. You will be part of a multidisciplinary team that includes specialists, nurses, and other healthcare professionals. Our team is dedicated to providing exceptional patient care and improving health outcomes.

Responsibilities
  • Examine patients, obtain medical histories, and treat acute and chronic illnesses
  • Order, perform, and interpret diagnostic tests
  • Involve patients and their families in care and treatment decisions
  • Create treatment plans and complete orders in patient charts
  • Collaborate on improvements in care delivery and service with staff, other physicians, and management
  • Initiate appropriate specialty referrals
  • Provide clinical information to the interdisciplinary team in a clear and timely manner
  • Ensure that proper patient documentation is timely, accurate, and complete
  • Work with practice management to ensure proper compliance with billing, coding, and CPT code assignment
  • Achieve and maintain optimal patient access
  • Provide interdepartmental medical consultation as requested
  • Participate in the development of medical protocols, standard procedures, and management of patient-care-related activities
  • Provide on-call coverage
  • Give feedback to and accept feedback from all staff in a respectful manner
  • Act in a positive and professional manner, complying with practice policies, procedures, and federal and state laws
Qualifications

We are seeking a board-certified or board-eligible Internal Medicine or Family Medicine physician with a strong clinical background and excellent communication skills. The ideal candidate will have experience in primary care and a passion for delivering high-quality patient care.
