Youth Support Specialist

About the Role:
Seven Hills Foundation is seeking a dedicated and compassionate Youth Support Worker to join our team. As a Youth Support Worker, you will play a vital role in providing home and community-based support, skill building, and case management for youth involved with the Department of Children and Families.
Key Responsibilities:
- Build strong relationships with youth in the community, focusing on social skills development, transition to adulthood, mental health coping skills, and family challenges.
- Work with youth of various ages who require increased support, including those with mental health challenges, family conflict, trauma, and educational needs.
- Assist youth in setting goals for work, career, and education, and provide support in achieving these objectives.
- Travel to various towns in Central Massachusetts to provide in-home and community-based activities, as well as meetings at the Department of Children and Families and other local agencies.
- Conduct needs assessments and develop individualized treatment plans to meet the unique needs of each youth.
- Assess the need for long-term supportive services and assist youth in obtaining referrals.
- Initiate referrals to therapeutic services, including substance use treatment, domestic violence services, psychiatric treatment, and long-term mental health services.
- Maintain close collaboration with other service providers to ensure coordination of care.
- Provide crisis intervention management and ongoing safety planning.
- Attend regular meetings with DCF to collaborate on treatment planning and ensure progress towards family goals.
- Provide ongoing supportive and/or case management services according to the identified needs in the treatment plan to assist youth in meeting their goals.
- Document all services and collateral contact made in a timely manner and maintain the electronic health record in accordance with agency standards.
Requirements:
- Bachelor's degree in psychology, social work, criminal justice, or other human service field.
- At least two years of experience working with youth and families, preferably in a mental health or social services setting.
- Valid driver's license and reliable transportation.
- Strong knowledge of child/youth mental health, family systems theory/practice, behavior management, and experience working with youth and families.
- Bilingual (English/Spanish) and/or bi-cultural background and/or skills are preferred.
- Ability to perform sedentary aspects of the position, including desk and computer work, typing, driving to meetings and trainings, and driving/transporting clients as necessary.
- Ability to perform therapeutic activities with children and families.
- Ability to perform duties with or without a reasonable accommodation.
- Employee may be required to intervene in order to de-escalate and/or provide safety of clients using techniques as trained and approved by Seven Hills Foundation, and appropriate to community and home-based settings.
