In this role, you will play a crucial part in delivering uplifting in-home care for seniors and older adults who need assistance, companionship, and daily support within their residences. Key Responsibilities:
The Client Care Coordinator (CCC) is responsible for overseeing the client journey from initial referral to ongoing care management. The CCC's responsibilities include:
- Collaborating with the Operations Manager to cultivate and maintain relationships with referral sources, including assisted living facilities, hospice services, geriatric care managers, and medical specialists.
- Engaging with potential clients by handling referral inquiries, providing information about our services, and arranging in-home assessments.
- Ensuring seamless care for clients by coordinating family resources and liaising with community providers, insurance companies, case managers, and other relevant parties.
- Working closely with the Homecare Services Coordinator (HSC) to align caregiver qualifications with client needs for optimal service delivery.
Qualifications:
- High school diploma or GED is required.
- One to three years of relevant experience in a similar role.
- Proven sales experience with a successful track record.
- Demonstrated leadership abilities and exceptional people management skills.
- Outstanding customer service and communication skills, both written and verbal.
- Willingness to spend significant time in the field, with driving as a requirement.
- A reliable vehicle and valid driver's license, with the ability to pass a background check.
Work Environment and Physical Requirements:
- Office setting.
- Physical activities include walking, sitting, and standing, with lifting generally limited to files and records not exceeding 5 pounds.
- Extended periods at a computer and on the phone.
- Time spent in a vehicle is also expected.
