WORK ENVIRONMENT: Remote

POSITION SUMMARY

The CCA Case Manager plays a crucial role in the coordination of care for clients enrolled in the Assisted Living Waiver Program (ALWP) under the Care Coordination Agency (CCA). This position involves assessing, organizing, and monitoring the needs of ALWP clients to ensure they receive appropriate services and support.

QUALIFICATIONS
  • Bachelor's Degree in social work, psychology, counseling, rehabilitation, gerontology, or sociology, along with a minimum of one year of relevant experience.
  • Current CPR certification is required.
  • Familiarity with California's Assisted Living Waiver Care Coordination Agency guidelines and provider handbooks.
  • Preferred experience in education, training, quality assurance, and management.
  • Demonstrated clinical judgment and proficiency in clinical practices, analytical skills, and documentation.
  • Strong organizational abilities and self-motivation with minimal supervision.
  • Proficient in computer usage and electronic information systems, with excellent communication skills.
  • Knowledge of Title 22 regulations.
  • Must possess a valid driver's license and have access to a reliable, insured vehicle.
KEY RESPONSIBILITIES
  1. Verify Medi-Cal eligibility for clients on a monthly basis.
  2. Schedule and conduct monthly client visits.
  3. Engage with clients to assess their needs, including medical equipment, appointments, and medication compliance.
  4. Collaborate with facility staff to ensure proper documentation and follow-up on incidents.
  5. Maintain accurate records of visits and client interactions, ensuring timely documentation in the system.
  6. Monitor client transitions in and out of facilities, notifying the CCA Coordinator as necessary.
  7. Facilitate the re-assessment process by coordinating with nursing staff and ensuring client signatures on required documents.
  8. Adhere to confidentiality, HIPAA regulations, and healthcare laws.
  9. Maintain up-to-date credentials and comply with company policies and procedures.
  10. Participate in departmental meetings and state-mandated training sessions.
  11. Perform additional duties as assigned.
