Registered Nurse Case Manager

Job Summary:
Del Sol Medical Center is seeking a skilled Registered Nurse Case Manager to join our team. As a key member of our healthcare family, you will play a vital role in promoting patient-centered care by coordinating the plan of care for patients, managing length of stay, and ensuring appropriate resource management.
Responsibilities:
- Provide program orientation to patients, families, and caregivers, including the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
- Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
- Ensure the implementation of the patient's treatment plan, supporting the patient's strengths, abilities, needs, and preferences.
- Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, or caregiver.
- Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
Requirements:
- Associate Degree in Nursing or Nursing Diploma Required
- Bachelor's Degree in Nursing Preferred
- 2+ years experience in case management OR 3+ years experience in clinical nursing Required
- InterQual experience Preferred
- Certification in Case Management Preferred
- Ability to establish and maintain collaborative and effective working relationships
- Ability to communicate effectively in oral, written, and electronic formats
- Demonstrates analytical and critical thinking abilities with proactive decision-making and negotiation skills
