Your responsibilities will include working closely with patients, medical providers, and other stakeholders to ensure seamless care coordination.

Key Responsibilities:

  • Provide medical case management services to individuals through in-person and telephonic communications.
  • Collaborate with patients, physicians, and other healthcare providers to develop and implement effective treatment plans.
  • Evaluate treatment plans for appropriateness, medical necessity, and cost-effectiveness.
  • Assess patient progress and make recommendations for improvement.
  • Attend medical appointments, hospital visits, and other meetings as needed.

Requirements:

  • Graduate of an accredited school of nursing.
  • Current RN licensure in the state of operation.
  • Certification as a CCM, CIRS, or other case management certifications preferred.
  • Valid driver's license, reliable transportation, and ability to travel to assigned locations.
