About the Role

We are seeking a dedicated and compassionate Care Coordinator to join our team in Philadelphia, PA. As a key member of our support staff, you will play a vital role in ensuring the safety and well-being of individuals with intellectual disabilities.

Job Summary

The Care Coordinator is responsible for providing supervision and support to individuals in accordance with their individualized service plans. This includes promoting healthy behaviors, encouraging social participation, and assisting with daily living activities.

Key Responsibilities
  • Provide one-on-one support to individuals with intellectual disabilities, focusing on their unique needs and goals.
  • Develop and implement personalized support plans, working closely with clinical staff and other stakeholders.
  • Assist individuals with daily living tasks, such as meal preparation, hygiene, and toileting.
  • Transport individuals to community activities, appointments, and social events.
  • Document progress and maintain accurate records, adhering to regulatory requirements.
Requirements
  • Bachelor's degree in a relevant field (human services, psychology, social work, etc.) or equivalent experience.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and colleagues.
  • Prior experience in human services or a related field, preferably with individuals with intellectual disabilities.
  • Valid driver's license and reliable transportation.
