RHD is a national human services organization dedicated to empowering individuals of all abilities. We are seeking a Support Coordinator to join our team in this challenging and rewarding role.

Job Summary

The Direct Support Professional, also known as the Support Coordinator, plays a critical role in ensuring the safety and well-being of individuals with intellectual disabilities and behavioral health histories. This position involves coordinating with clinical staff, implementing residentially based interventions, and assisting clients in achieving their clinical goals.

Key Responsibilities:
  • Provide individualized support and supervision to clients in accordance with their Person-Centered ISP, promoting healthy and socially acceptable behaviors.
  • Complete required documentation, including progress notes and incident reports.
  • Administer medication as prescribed by medical professionals.
  • Transport clients to appointments and activities in the community.
  • Assist clients with daily living tasks, such as meal preparation, toileting, and hygiene.
  • Participate in Community Integration Activities and access community resources, including medical appointments and social security benefits.
  • Perform miscellaneous tasks as assigned by supervisors, including administrative duties and training new staff members.
Requirements
  • Bachelor's degree in Human Services or related field preferred.
  • High school diploma or equivalent required.
  • Basic computer skills and internet literacy necessary for data entry and program management.
  • Prior experience in the human services field, preferably in direct support or a related role.
  • Valid driver's license and ability to obtain additional certifications as required.
Salary and Benefits

We offer a competitive salary range of $40,000 - $50,000 per year, depending on experience and qualifications.
