Job Description

The Site Manager will assist with day-to-day management of participants in our Developmental Intellectual Disabilities program. This role requires a strong background in intellectual developmental disability and autism, with a minimum of 4 years of supervisory experience.

Salary: $43,680.00 annual salary or $21.00/HR

Key Responsibilities
  • Coordinate and/or complete initial, annual, and revision consumer assessments for assigned caseloads.
  • Identify in the assessment the consumer's requirement for direct supervision or times when the consumer may be unsupervised.
  • Disseminate the assessment for the development of the IHP, IHP annual update, and all IHP revisions to all IHP plan team members.
  • Provide training to Direct Support Professionals on the consumer Support Plan if a medication is prescribed to treat symptoms of a diagnosed psychiatric illness prior to the Direct Support Professional working with the consumer.
  • Participate in the development of the IHP, IHP Annual Updates, and all IHP revisions.
  • Fulfill the role of Plan lead when a Program Director is not available.
  • Complete quarterly reviews every 3 months (or more often if necessary).
  • Report IHP content discrepancies to the Program Director as applicable and Team members.
  • Implement the IHP as written.
  • Supervise, monitor, and evaluate services and outcomes identified in the IHP.
  • Supervise all DSP members/agency staff.
  • Review, sign, and date the monthly documentation of an individual's participation and progress toward outcomes.
  • Review all incident reports and sign all incident reports once completed and ensuring that the Program Director reviews each incident report.
  • Report a change related to a consumer's needs to the Program Director as applicable and to plan team members.
  • Complete all DSP working schedules.
  • Recommend a revision (addition, deletion, modification) to the service or outcome in the IHP when needed.
  • Coordinate the services provided to a consumer.
  • Coordinate the training of Direct Support Professionals in the content of health and safety needs relevant to the consumers on your caseload.
  • Develop and implement component services that will lead to advancement Active Treatment.
  • Assure all facility van, physical site, and trainings are in compliance with DOH 6600 regulations.
  • Adhere to HIRS (Health Information Record Systems) standards in completing Medical and Programmatic Books.
