About the Role

We are seeking a highly skilled and organized Patient Services Coordinator to join our team at CenterWell Home Health. As a key member of our care coordination team, you will play a vital role in ensuring the seamless delivery of high-quality patient care.

Key Responsibilities
  • Coordinate patient visits and schedules, ensuring timely and efficient delivery of care.
  • Communicate effectively with field staff, patients, physicians, and other stakeholders to maintain proper care coordination and continuity of care.
  • Assist with day-to-day office and staff management, including managing schedules, editing schedules for agents calling in sick, and updating agent unavailability in the worker console.
  • Initiate infection control forms as needed and send completed reports to the HR department.
  • Serve as a backup during busy times, including receiving calls from the field staff and assisting with weekly case conferences.
  • Refer clinical questions to the Branch Director as necessary.
  • Maintain the client hospitalization log, including entering coordination notes and sending electronic logs to all office, field, and sales staff.
  • Complete requested schedules as tasks appear on the action screen.
  • Ensure staff are scheduled for skilled nurse/injection visits unless an aide supervisory visit is scheduled in conjunction with the injection visit.
  • Schedule discharge visits/OASIS collection or recert visits following case conferences when tasks appear on the action screen.
  • Schedule TIF OASIS collection visits and delete remaining schedules.
  • Reschedule declined or missed visits (if appropriate).
  • Process reassigned and rescheduled visits.
  • Ensure supervisory visits are scheduled.
  • Run all scheduling reports, including the Agent Summary Report and Missed Visits Done on Paper Report.
  • Prepare weekly Agent Schedules.
  • Perform initial review of weekly schedules for productivity/geographic issues and forward schedules to the Branch Director for approval prior to distribution to staff.
  • Verify visit paper notes in the scheduling console as needed.
  • Assist with internal transfers of patients between branch offices.
  • Initiate Employee/Patient Infection Reports as necessary.
  • May be required to perform patient visits and/or participate in on-call rotation (clinical role).
Requirements
  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Excellent communication skills, ability to interact well with a diverse group of individuals, strong organizational skills, and ability to manage and prioritize multiple assignments.
  • At least 1 year of home health experience.
  • Prior packet review/QI experience preferred.
  • Coding certification is preferred.
  • Valid state driver's license and automobile liability insurance.
  • Currently licensed in the State of employment, if applicable.
