Position Overview

M Health Fairview Health Services is seeking a dedicated and motivated Registered Nurse to join our dynamic Care Management Department.

As a key player in our team, you will be based at our Edina, Minnesota campus, providing essential support to individuals in the southwest Twin Cities metro area.

Our facility offers a wide range of specialty services, blending advanced technology with compassionate care to meet the diverse needs of our patients and their families.


Unit Highlights:
The RN Care Transition Specialist is responsible for delivering comprehensive care coordination for assigned patients.

This role involves assessing patient care plans, developing and implementing strategies, and monitoring progress to ensure optimal resource utilization and patient outcomes.

The level of care coordination is tailored to the specific needs of each patient and the requirements set by payers.

This position is accountable for the quality of clinical services provided, identifying and addressing any barriers that may affect patient care.


Responsibilities include determining the best approach to fulfill duties within established protocols, consulting with leadership on any atypical situations.

Internal stakeholders encompass all levels of nursing management, medical staff, and interdisciplinary healthcare team members, while external stakeholders include physicians, payers, community organizations, and regulatory bodies.


Work Schedule:
This role requires 64 hours per pay period, with day shifts of 8 hours and one weekend commitment per month. We offer a comprehensive benefits package, including medical, dental, and paid time off.

Key Responsibilities:


The Registered Nurse will be responsible for:

  • Assessing, planning, delivering, and evaluating nursing care for designated patient populations.
  • Coordinating care with healthcare team members and delegating tasks as appropriate.
  • Managing patients throughout the healthcare continuum to achieve optimal clinical, financial, and satisfaction outcomes.
  • Serving as a primary contact for patients, physicians, and care providers during hospital stays.
  • Implementing transition functions and activities to ensure seamless patient experiences.
  • Assessing patient admissions and ongoing stays using evidence-based criteria.
  • Collaborating with healthcare partners to develop and execute individualized discharge plans.
  • Facilitating effective communication of care plans across the continuum.
  • Identifying and mitigating delays in care to promote safe and effective discharges.
  • Collecting necessary data for payer requirements and regulatory compliance.
  • Establishing collaborative relationships with physicians, medical directors, and nursing staff.
  • Mentoring team members on case management principles.
  • Tracking outcomes and participating in quality improvement initiatives.

Qualifications:
Required:
  • Current MN RN License
  • BLS Certification for Healthcare Providers
  • Three to five years of recent acute care clinical experience
Preferred:
  • One or more years of experience in care coordination or case management
  • Bachelor's Degree in Nursing
